This document is used when you need medical treatment, but are too sick to express your treatment wishes to the doctor. The living will portion of the document lists the types of medical treatment you want and those you don’t want. The power of attorney portion of the document allows you to appoint someone who can make medical decisions for you, in case a treatment is not covered by the living will.
